What is Breast Augmentation?

Breast augmentation surgery, or augmentation mammoplasty, is a surgical procedure that uses medical-grade implants or autologous fat grafting to improve the size and contour of the breast. During the breast augmentation procedure, we create a breast pocket either behind the glandular tissue or partially beneath the pectoral muscle to house the implant.

We offer both high-cohesive silicone gel implants, which have a consistency similar to Jell-O or gelatin, and traditional saline implants. Dr. Sukkar uses specific techniques to minimize trauma to the blood vessels and blood supply in the area, which helps the body heal and ensures the implants sit naturally against the chest wall.

People often use these terms to mean the same thing, but breast augmentation is the goal of increasing size, while breast implants are one tool that can be used to get there. Most women choose an implant to reach their target breast volume, but it isn't the only way to add volume. We look at your anatomy and goals to decide if a medical device or your own tissue is the better fit.

Fat Transfer (Fat Grafting)

For those seeking a very subtle change without using artificial materials, a fat graft is an excellent option. We use a process similar to a fat transfer to other areas of the body to harvest fat from areas like the hips or thighs, process those cells, and carefully inject them into the breast. This creates a natural appearance with no risk of implant rupture, though it generally provides a more modest increase in volume compared to traditional breast surgery.

Types of Breast Implants

There are a lot of different implant types, and we use what we call a biodimensional approach to select the right one. We don't just focus on CCs; we take measurements of your chest to decide what works from a dimensional standpoint.

Saline Breast Implants

Saline implants consist of a silicone shell filled with sterile saltwater. They are very durable, we’ve seen them last over 30 years, and if a leak occurs, your body safely absorbs the saltwater. One thing to consider is that because saline is a liquid, it isn't quite as natural-feeling as silicone. It can also be more prone to visible implant rippling if you don't have much breast tissue for soft tissue coverage.

Silicone Breast Implants

Probably 95% of the patients in our practice choose silicone implants. These use a viscous silicone gel that looks and feels very much like our normal tissues. Unlike the older jam-like implants from the 80s, these newer "gummy bear implants" hold their shape like gelatin. They are less likely to ripple and provide a very discreet, soft result.

Size, Shape, and Profile

We determine the circle size of the implant first based on your measurements, then we determine the projection and profile. Round implants typically provide the most fullness in the upper part of the breast. Shaped implants offer a more sloped, anatomical contour. You can choose between a smooth surface or a textured surface, though Dr. Sukkar will explain how each interacts with scar tissue and the immune system.

Your breast implant surgery takes place in our private, accredited center serving the Galveston area. You will be under general anesthesia (or occasionally local anesthesia for minor revisions) managed by a professional team. Dr. Sukkar almost exclusively uses the inframammary fold incision (the crease under the breast).

He prefers this approach because it doesn't disturb the breast tissue and allows him to get immediately under the pectoralis major muscle. Multiple studies prove this incision has the least chance of bacterial contamination compared to the armpit area or nipple areas, where bacteria concentrations are higher. It also protects the milk ducts, which is important for those who may wish to breastfeed later. Afterward, you’ll rest in a private suite with our nursing staff.

Most of our Galveston patients get back to their normal daily routine within three to five days. You’ll wear a supportive sports bra for about six weeks to keep the implants in the right position while you heal. All procedures have potential risks, such as capsular contracture (tight scar tissue) or implant rupture. There is also a very rare risk known as anaplastic large cell lymphoma (BIA-ALCL). We will go over all of this with you.

Yes. If you ever want your breast implants removed, we can do that. Some women choose to replace the volume with fat transfer using their own tissue.

You can still have imaging done. You just need to let the technician know you have implants so they can use special views to see all the tissue clearly.

If you experience unusual breast pain or suspect a breast implant illness, we recommend an immediate follow-up. We can use our in-office ultrasound to tell if the silicone breast is intact.
