Breast implant sizes are measured in cubic centimeters, or cc. Standard implants often range from 200cc to 600cc. Implants above 600cc are typically considered extra large.
Many clinics classify implants as “XL” when they reach 700cc or more. These implants can go much larger, with some models reaching as high as 3000cc, especially for patients who want a dramatic increase in volume.
Extra large implants can be saline or silicone, but very large ones (usually over 1000cc) are often saline because this allows for more flexibility in size during surgery. Surgeons will consider a person’s body frame, tissue quality, and desired outcome when deciding how large they can safely go.

Extra large breast implants come in a few main types. Each breast implant type uses different materials and design features that affect how the breasts look, feel, and fit the body’s needs.
Saline implants have an outer shell made of silicone. The shell is filled with sterile salt water during surgery. One big advantage is that saline implants can be adjusted easily by adding or removing fluid during placement. For extra large implants, saline is the main choice because it allows sizes up to 3000cc or even more.
Saline implants are often recommended when a very large size is requested. They can be placed with a smaller incision since they are filled after insertion. In the rare case of a rupture, the saltwater is safely absorbed by the body. Their main drawback is that they may feel less natural than silicone gel, especially in thinner patients.
Silicone implants use a silicone shell filled with a soft, cohesive silicone gel. These are very popular for most breast augmentations because they are soft and feel more like natural breast tissue.
For extra large sizes, though, silicone gel implants are not as common because they have an upper size limit, usually around 800cc. Safety regulations and risks from heavier, solid gel filling limit how big these implants can get. For patients who want dramatic results, this can be a drawback.
Most people need to take it easy and rest after surgery. Doctors usually recommend a lot of bed rest for the first few days and wearing a surgical bra. Pain, tightness, and swelling are common at first, and light walking is important to help prevent blood clots. Most patients resume normal activities in about two weeks. Avoid heavy lifting and intense exercise for at least six weeks.
As with any major surgery, risks can include infection, bleeding, and problems with healing. People with extra large implants may also face issues like skin stretching, changes in nipple sensation, implant shifting, or excess scarring. Larger implants can put more strain on breast tissue. Over time, this can cause drooping or back pain.
The final cup size after breast augmentation depends on the starting breast volume, shape, and the size of the implant chosen. Implants over 800 cc are considered extra large and can give a very dramatic change in bust size. Implants sized at 1500 cc or 2000 cc can increase the bust by several cup sizes, but results will vary based on body type.
The maximum implant size depends on factors like chest width, skin stretch, and the amount of breast tissue. The FDA approves silicone gel implants up to 800 cc, but saline implants can be overfilled past that size to reach 1500 cc or even 2000 cc, under certain conditions. Surgeons consider each person’s body to decide what is safe. Stretching the skin too much or using a size that’s too large for the chest can lead to more risks.
